Attendance Certificates, CME Credits

We are pleased to inform you that 16th International Celiac Disease Symposium (Prague, Czech Republic, 21.–24.06.2015) was granted 15 European CME credits (ECMEC) by the European Accreditation Council for Continuing Medical Education (EACCME).

Certificates of attendance will be distributed at the registration desk. Date and time will be announced soon. Please do not forget to pick up your certificate; only e-mail vision can be send after the symposium.

How to register

To register for the Symposium, please fill in the ON-LINE Registration Form

Please note that the ON-LINE Registration Form is only accessible with a password. Simply fill in the Personal Data Form and a password will be sent to you by e-mail. Your password could be used for your payment, accommodation reservation and for booking other services.

Upon receipt of the Registration Form and the payment, a Letter of Confirmation will be sent to the participants within three working days. Please present the Letter of Confirmation at the Registration desk at the Symposium venue to pick up your materials.

Early registration fees will only be confirmed if the registration form and the payment are received by the Sympsoium Secretariat by May 7, 2015.

Registration forms received after June 14, 2015 will not be accepted as pre-registration. Please do not send any Registration Forms or payments after that date. You will have the possibility to register in the Symposium venue. The on-site registration will be available from June 21, 2015.

  • Coffee breaks and lunches during the symposium


Clinical Forum Participant and Student registration fee includes

  • Admission to all sessions of the Clinical Forum and Patient´s Forum ONLY
  • Admission to the Opening Reception on Sunday, June 21, 2015
  • Admission to poster and exhibition area
  • Symposium materials (delegate bag, final program etc.)
  • Coffee breaks and lunches during the symposium


Patient´s Forum Participant and Student registration fee includes

  • Admission to all sessions of the Patient´s Forum and Clinical Forum ONLY
  • Admission to the Opening Reception on Sunday, June 21, 2015
  • Admission to poster and exhibition area
  • Symposium materials (delegate bag, final program etc.)
  • Coffee breaks and lunches during the symposium


Accompanying Person registration fee includes

  • Admission to the Opening Reception on Sunday, June 21, 2015
  • Grand City Tour of Prague (3,5 hrs) - Monday, June 22, 2015 at 14:00
  • Coffee breaks and lunches during the symposium


Visa Requirements

Invitation letters for visa purposes can only be sent to people who have completed the registration process. Further information regarding the visa requirements you can find HERE.
 

Cancellations and refunds

The Symposium Secretariat must be notified in writing by e-mail (icds2015@guarant.cz) or fax (+420 284 001 448) about the cancellation of the registration. Telephone cancellation will not be accepted. The appropriate refunds will be made after the Symposium.


The following cancellation conditions apply:

Cancellations received Refund
before May 7 ,2015 full refund of the registration fee less 30 EUR handling fee
May 8 – May 31, 2015 50% of the registration fee
from June 1, 2015 no refund

 


Methods of payment

Registration fees must be paid in EUR by one of the following methods:

1. Credit Card

For payments through the Online Payment System only MasterCard/Eurocard, Visa and Diners Club credit cards are accepted. These payments will be charged in EUR.

For payments using American Express credit cards please complete the Credit Card Payment Authorization Form and kindly send it signed by a card holder to the Symposium Secretariat by fax (+420 284 001 448) not via e-mail for security reasons. The payment will be charged in local currency – Czech crowns (CZK) – at the exchange rate of the Czech National Bank valid on the date of payment.

Credit Card Payment Authorization Form can also be used for MasterCard/Eurocard, Visa (will be charged in EUR) and Diners Club (will be charged in CZK).

 

2. Bank Transfer

Please note that payments by bank transfer will only be accepted by June 8, 2015.  After this date only payments by credit cards will be accepted (either by using the Online Payment System or by submitting the Credit Card Payment Authorization Form as mentioned above).
